Halifax Rainbow Community Health Care Survey

July 2007 to September 30, 2007

In an effort to assess and improve the health care needs of the rainbow 
community, the Capital Health and IWK Health Centre, GLBTI Initiative 
has developed the following questionnaire. Our plan is to use this 
information to inform the new model of care we are working on. The 
results of this survey will make a difference for everyone in our 
community. Our aim is to ensure that every community is our community. 
We thank you for you time.

Our success means better access to health care for you.
