When I rented from AVIS last weekend in Oregon, I had to decide on four
insurance clauses totally $20 per day.  The biggest one was for damage to
the car, then there was one for medical, and yet another one for revenue loss
to AVIS while the car is out of commission.  The last one is a new twist
and not covered by my regular auto insurance.  It says that if it takes
two weeks for AVIS to repair the car, then you owe AVIS two weeks of rental
fees (at the highest daily business rate I'd assume).  It is not uncommon for
the insurance cost to exceed the basic rental rate (e.g. $140 / week for
insurance and $99 for rental.)

Also read your clauses on your credit cards that provide insurance coverage.
There are lots of exceptions.  For example, AMEX dropped coverage on its
green cards, but maintains on its gold cards and perhaps business cards.
Check to see if they cover more than the physical damage to the car too.
