One would presume that most of these costs are part of the $40k/yr.
Certainly the medical costs are.

Completely irrelevant.  As inflation consists of increasing prices
and wages, the number of dollars collected from both sales and
income taxes increase.  So the future costs are indeed represented
by larger numbers but that's a case of dollars getting worth less
and less, not real costs getting higher.
