You can try out head tracking devices that don't require a mouse at all.
I have a mac system with a head tracker called a HeadMaster from PRC. It has
an ultrasound headset that you wear and a mouth tube to blow to click. I have
had trouble with it as it seems to send constant "mouseup" signals-- I am
using a Quadra with system 7 and this thing seems to be kind of old. But its
the right idea. Let me know if you find something like this.

You should ask Doug Engelbart about his early research with tracking
devices. He tried a very primitive head tracking system. If you don't
know, Doug invented the mouse, windows, menus, display editing and
implemented the first hypertext system. His address is
dce@sunrise.stanford.edu.
