When I was teaching, any student that wanted to challenge his or her
grade on any of my exams was allowed to, provided that their challenge
was submitted in writing. They were told, in the syllabus that I passed
out at the beginning of the term, that this included ANY re-grade
request, even if all they were claiming was that I added up the points
wrong. They were to state what their complaint was about the grade I'd
given them, and the remedy they were seeking. I explained that I had
established this procedure because it was the only way I knew that I'd
be able to keep track of everyone's request.

I never had any complaints about my grading when I followed this
procedure. The students knew that their complaints were being heard, and
whether they received a corrected score or I ruled that the original
score was correct, they understood why they got the grade they received.
Most changes, if there was a change, were minor, but at least one
student was able to prove to my satisfaction that I'd missed an
important part of his solution during my original grading.

I'm sure that this policy helped reduce the number of regrade requests
I received. I can't prove it, but I'm sure that some of the students,
while being forced to explain why my grade was wrong, found out that
they themselves were mistaken, and that my marks were correct.
