We need your help if you are a resident of the United States. (This is
a survey. It is NOT a sales promotion).

This is an important medical related survey focused on evaluating the
feasability of  a workable wellness program that has real and long term
value to it's participants. The purpose of this survey is to also get
feedback from you as to if a program that really helps in the
prevention of illness, desease, chronic conditions, provides motivation
for member participation and would be provided as a benefit included
with medical coverage without increasing  rates would be something that
would be actually used by the participants.Please invest a few minutes
of your time to provide information that may play an important part in
your future. If you could control the creation of your own personal 
wellness program, what tools would you include that would enhance 
and provide the most value and real usability?

To make it as simple and convenient as possible for you to respond to
this questionnaire,  please follow these instructions . The answers to
questions 1 through 4 will be coded with letters - a. b. c. etc..
Simply jot down on a peice of paper the question number and the
response letter to each question. Question 5, please indicate state
