Abstract
Many investigators of clinical reasoning in medicine presume that the
process can be understood by reference to an analytical representation
of medical knowledge (propositions, prototypes, or conditional
probabilities). The present research is based on an alternative view
that clinical reasoning in familiar situations is primarily based on
holistic similarity to previously encountered cases. The research
leans heavily on instance models of concept formation (associated with
Jacoby, Brooks, Medin and others). In contrast to some views of case
based reasoning, it is assumed that the similarity search is not
deliberative and analytical, thus may be influenced by features of
similarity which are analytically irrelevant. Evidence will be
presented from a variety of domains in medicine, primarily dermatology
and radiology, to establish the existence of this processing mode and
to explore influences on the dominance of analytic or non-analytic
processing.
