Where people have installed their browsers from ISP installation CDs 
this often happens. To get rid of it, you'll need to edit a registry key 
(or uninstall and reinstall the browser from another source).

Fire up regedit and search for "provided by Demon Internet" - change the 
value of the registry key to whatever you want the titlebar to say. This 
MIGHT be in more than one place, so keep searching after you've found 
the first one.

Obviously be careful not to mess up your registry. :)

And as the other poster said, to put your name in the titlebar you want 
to use the HEAD tag.
