TOC entries for 1st page in chapter misplaced to end of previous chapter

My chapters had 2 text flows. The 1st contained just the chapter title with no side heads and the 2nd the main text with sideheads. 

The generated table of contents puts headings from the first page of a chapter at the end of the previous chapter's entries. Heading entries after page one of a chapter appear in the correct chapter. Other than this, I have a great looking TOC. The reference pages like fine and all the styles have the TOC suffix. 

Example: 

Chapter 1: XXXXX                page 1-1    YYYYY                        page 1-2    TTTTT                        page 1-5    ZZZZZ                        page 2-1 

Chapter 2: XXXXX                page 2-1    YYYYY                        page 2-2 

Figuring this might be a text flow problem, I combined the 2 text flows into one and this problem disappeared. 

I'd like to get back to 2 text flows because of not wanting a side head in the chapter title flow. Any ideas on how to correct the misplaced entries?
