Having the individual files open doesn't matter, what matters is that any other books that are referenced by the book you are distilling are open and that you do not print to postscript using *.* or *.ps as the filename. I think the "Cannot find the file named xxx/xxxx/xx...xx/xxx.fm" error message is the clue--that looks like an absolute reference to a file on another drive. Is it possible that you have been moving files from one computer or drive to another, or have included a file from a network drive in your book, or anything along those lines?
