Two questions:

1. When you say 

      The Editor tried making changes to the INCA file while it is concurrently
      open on the Designer�s computer.




How is it that the Designer has an INCA file open? Does the designer use InCopy?

2. In Attempt 2 it sounds like the "live-edit" system works. How is it not working for you?

If possible, I think it would be worth while to do some testing on another server. Last summer a Novell patch to the Mac file services broke the live-edit plug-ins. Down grading the server to the previous version solved the problem.
