"[/PageMode /UseOutlines /DOCVIEW pdfmark" sets the initial view (when the created PDF is opened) to "Thumbnails and
page", but does not automatically generate the thumbnails. To create thumbnails, set the appropriate option in
Distiller; you can also use "<</DoThumbnails true>>setdistillerparams". See the Acrobat SDK Docs on "Acrobat Distiller
Parameters" for details. Also note that one of the options in Distiller is to lock the various parameters (and so
settings coming from the PostScript file may be ignored...).

No idea about the "firts page number".
