changing PageWidth and PageLength - where does it go?

I'm using C++ and the DEVMODE structure to attempt to change the PageLength and PageWidth to custom settings for printing a set of docs. Normally, this would appear in the structure, however, it appears the Adobe PS Drivers are storing these values elsewhere. I can see choosing "Custom Postscript Page Size" sets the PageSize to MAX_INT.

Can anyone tell me how to update these values at the driver level (so they become system-defaults)?

Thanks!
