Browser Based Review

I need to get the browser base review process working. I have a PDF doc created with the "enable for commenting and analysis in acrobat reader".

When the review is started and loaded into Safari, I'm getting a error message "failed to open page ( See Activity window for details). The activity window indicates "Plug-in Cancelled".

The tool bar for commenting and review is not loading.

Any ideas on how I might fix this? Or where I can get more information on browser based review?
