Hello,

I would like to take the pdf form located here:
 <http://www.ctbwireless.com/pdfs/CTB%20Account%20Application.pdf>
and adjust it so that I can make the form fillable, and also make it so the user can submit the filled out pdf form automatically (with a submit button or something). The file would then ideally be sent as an attachment to the corporate e-mail. Could someone please help me with this?

Do I need to make a new PDF form?
What Adobe software will do all of this? (including the e-mail submission)
