Want to be able to combine static PDF pages with a fillable form

I need to be able to fill out a form (check request) and then add additional pages that are the backup to said check request.

Professional informs me that this a XML form and I cannot do that.

Is there a way to make it a "static' form or other image type form that I can then use as a regular PDF that I can add and remove pages from?
