Unfortunately you can't use the index of an item or the label directly, but
you can use the label like this:

set myMainFrame to master page item 1 whose label is "Main box"

You can also use the id:

tell application "InDesign 2.0.1"
 tell document 1
set n to id of text frame "Main" of master spread 1
set theFrame to master page item id n of page 1
  -- do stuff to it
 end tell
end tell

The real difference from QuarkXPress is not a scripting issue so much as the
quite different ways they implement master pages and items on them. Both
methods have their advantages and disadvantages, but I don't think ID's
method has a lot to offer the scripter.

In fact, there's something to be said for having items you intend to edit on
another master page altogether, and just create items as you need them, like
so:

tell application "InDesign 2.0.1"
 tell document 1
set theProps to properties of text frame "Main" of master spread 2
make text frame at page 3 with properties theProps
 end tell
end tell

This can make it easier to keep track of an item's layering, especially on
pages where there are multiple layers.
