I think the problem has been created by our layout programs. In the old days (pre computerized layout), there was no problem. We measured the page size and margins in whatever measurement system paper suppliers and printers used, by country. Set up our columns in point/picas. Specc'ed our type in points/picas. It's only since layout programs don't recognize the difference between the page measurements and type measurements that we have the issue. I recently asked for the abilty to have separate page and type measuring systems in a document as a feature request on the InDesign forum. Maybe more of us could ask for this in our respective layout programs.
