#include IdAuthenticationDigest.hpp into your code.  Since you are in a C++ 
environment, you may also have to call RegisterAuthenticationMethod() 
manually at program startup as well:

    RegisterAuthenticationMethod("Digest", 
__classid(TIdDigestAuthentication));

It does so automatically after TIdAuthenticationDigest has been registered.
