How about using a JFileChooser and customizing it a bit.

You can set the text on the buttons fairly easily, and you could use
the setAccessory(JComponent) method to add your checkboxes etc. I'm
guessing this may well suit your needs without you having to totally
re-layout a file chooser again.

(Which probably isn't too bad but think about getting all the drive
info etc, its more work than it needs to be!)
