Actully the best solution would be to get the IBM courier type 1
font, which was made public by IBM. It's available as file
tex/courier.tar.Z on ftp.urc.tue.nl (131.155.2.79). You'd have to
reencode the font to get the IBM-PC code-page 437 character mapping,
but all the characters are already there in the font (sometimes with
strange character names; for example the line drawing characters have
been given the names /SF010000 to /SF540000 -- there's actually a
table of these /S??????? names in the back of the UNICODE book by
Addison-Wesley).
