Are you using the latest Creative Labs drivers, or the latest reference drivers from Permedia?
What model Exxtreme card is it?
What make and model motherboard?
Also, what operating system are you using?

Make sure you have the latest BIOS and drivers for your motherboard.

Try booting into SafeMode and removing all entries in Device Manager under Display Adapter and reinstall the card. Make sure
there are no drivers loading for any other video display.
