What explains the fact that almost 100% of those "violations" 
result in no accident?

When I drove the Massachusetts 128 "circle" to work every day
for 7 months a few years ago, I saw ONE accident.  In that same
time there must have been 100,000 cars on that road at the same
time as me.

That's 99.999%.
