I'm sorry if this insults you but did you make a root disk as well?  One 
suggestion I could make is to make another bootdisk image from a file off
a WWW site.  I borrowed a CD from someone here at work and two of the files
were invalid.  I got the same corresponding files from a WWW site and it
works great now.  Also when I first tried installing it with my boot and root
disks the setup wouldn't run.  But I got a new rootdisk image from that same
CD and it just fired up after that.  (Now if I can only get the kernel image
compiled for my SB16 and mounted on the MBR and get X to work...)
