Thank you, Steve, for providing this information. As part of the new
FACT Act (Fair and Accurate Credit Transactions Act),
Equifax/Experian/Transunion are required to provide 1 free credit
report, and MIB is required to provide 1 free medical report.

For the credit report, FTC has designed a simple request form that
requires (name, DOB, SSN and address). I wonder why they did not design
the form for request from MIB.

1. MIB does NOT allow request by mail, or web, only by phone.
2. MIB's request requires name, DOB, SSN, address PLUS occupation,
phone number, etc. All the "PLUS" information is of course superfluous,
and they collect so they can sell it for more money.

Any one has success getting medical report from MIB with by only
providing the minimal information?
