We're studying the feasibility of using the World Wide Web to gather
health-status information.  We've developed a Web version of the RAND-36
-- it has the same questions as the Medical Outcomes Study (MOS) SF-36
"Short Form" with a simplified scoring system.

The survey is available at:

We encourage you to try out the survey -- it's fast, easy, and
confidential.  Once you fill out the form, the server computes your scores
and shows how you compare to the MOS study's population of almost 2,500
patients.
