At a guess you've missed the step where you modify inetd -- qmail doesn't run
as a monolithic programe like sendmail. it has another program which receives
messages and passes them to qmail-send to sort and deliver.
Nick Leverton's got a really neat patch for qmail which controls delivery
when you go online.

from qmail-1.01/install:
23. Set up qmail-smtpd in /etc/inetd.conf (all on one line):
smtp stream tcp nowait qmaild /var/qmail/bin/tcp-env tcp-env /var/qmail/bin/qmail-smtpd
Also comment out comsat in /etc/inetd.conf.
24. Reboot. (Or kill -HUP your inetd and make sure the qmail daemons
are running.)
check this is working by doing:
$ telnet 0 25
which should produce something like:
Trying 0.0.0.0...
Connected to 0.
Escape character is '^]'.
220 illuin.demon.co.uk ESMTP
type:
quit
to end the connection.
221 illuin.demon.co.uk
Connection closed by foreign host.
[0.0.0.0 means 'any local address' and typing just 0 sames the ole fingers]
